In general terms, no, it doesn't matter. The umpire has to be told (often so he can get out of the way if it's a spinner) and he'll announce the change, which can happen at any time during the over - it doesn't have to be right from the start. Technically it can make all the difference; altering the angle the ball so it comes into a left-hander for instance, taking it away from a right-hander, utilising the rough for a spinner and so forth.
