So i know this has been done to death and there is general answer as it depends on your workshop and equipment etc etc. I have a TS200-2 which i have put on a frame and added the big mouth on the bottom and sealed up in side ( I havent modded the blade cover yet - could be the issue?). I have a 4" pipe coming on to a reducer to my shop vac.....I have removed the over head extractor and cover as the cut i have been making needed them off.

However, i still have loads of dust kicking around and coming out. So my question is, do i need to upgrade my shop vac to an extractor and keep the the shop for hand tools?

I think you know the answer.
Yes
A circular saw need a very large air movement to properly clear the dust.
