I wouldn't even be attempting to screw into 12mm MDF you're just asking for trouble, it's fine screwing into 18mm and above but 12mm will be a nightmare. Why can't you just glue? Any p.v.a will be fine, cheap ever built stuff will work perfectly

A cut string staircase is different to that, the spindles sit directly onto the tread of the stairs. They're normally fixed with dowels or a mortice and tenon into the tread. It will be interesting to see pics of what has actually failed on this staircase?
Four years is not a long time, fourty years yes, but four years is nothing, I wouldn't expect my staircase joinery to fail in that time. Go fix it otherwise prepare for that customer to tarnish your reputation.
Yes hall clips to fasten it, I meant to seal it sorry. I don't think mortar is acceptable anymore but I may be wrong? I thought mortar could only be used on really wide joints and even then the top and bottom of the mortar where it meets the lead and the brick/stone had to be sealed with leadmate
