It does depend upon how uneven but I use a mix of chipboard sheet and another area of second hand bitumen backed floor tiles.
All straight down on the concrete, no problem moving anything and acts as a damp barrier.

Use no adhesive.
Dowel the sections together.
Make a suitable flat/decorative hub for the centre to receive and make captive the ends of all the sections.
The hub could be in two halves which clamp down upon the sections for adjustment and from the underside, provide an anchor to whatever sub...

Considering that the primary reason this repair has failed is a 80% poor glue joint I'd simply clean off the two flat surfaces and re-glue it.
If this is for someone else DON'T be screwing chunks of metal to the surface and expect to be paid because in anyone's book that's a first class bodge...
Mix this stuff with the sandings from your work and it creates a paste which sets really rapidly to a crispy hard finish.

ps I wouldn't be trying to get the boards too close to finished size before gluing up.
You could glue it up in 2 or 3 widths ( to fit over/under your planer)...
